The main activity of this team is the Hospital Admission Risk Program which provides services to clients with chronic illness and complex needs reduce avoidable hospital admissions through health interventions. KCHS is a lead partner in this initiative with Eastern Health and other community organisations and provides intake, dietetic and physiotherapy services.
