The main activity of this team is the Hospital Admission Risk Program which provides services to clients with chronic illness and complex needs reduce avoidable hospital admissions through health interventions. KCHS is the lead partner in this initiative with Eastern Health and other community organisations. KCHS directly employs staff to provide dietetic, physiotherapy, podiatry services as well as Community Nursing Care Co-ordination. KCHS also manages the contract for the provision of Allied Health Services and the Psychosocial Stream for Eastern HARP from staff employed by the Eastern Community Health Services.
