Intake Information & Referral

The intake service will be the first point of contact for people wanting to access community health services. Individuals will be asked a range of questions which will assist to determine the appropriate services that are required. All our Intake workers are all qualified practitioners and come from a variety of backgrounds: Welfare, Social Work, Nursing and Psychiatric Nursing.

Please note we are not an emergency response service.
If you need urgent medical assistance please call emergency services on 000 or contact your local doctor without delay.

How do you get an appointment at KCHS?
• You can call and speak to an intake worker.
• You can make the referral in person.
• We can receive a written referral from your doctor or referrer through the mail, fax or email.
• Interpreters can be arranged on request – please ask our staff for assistance with this service

Speaking to an Intake Worker
All new referrals to Knox Community Health are first assessed by the Intake Service. The Intake Workers role is to collect information about you, firstly to see if you are eligible for a service from us - we will ask you where you live and, as some services require a health care or pension card we may ask you to give us these details.

The Intake Worker will then spend some time discussing with you the details about your referral. This is important as it helps our clinicians understand the difficulties you are having and lets them know if you need a priority appointment. This process may also identify other services that you may find helpful.

At the end of the discussion the Intake worker may book an appointment for you, they may place you on a waitlist for an appointment (if there are none available in the next few weeks) or they may refer you to a more suitable service.

The Intake Service has access to information about other agencies and community services in the region. We can assist you to contact other agencies to access their service.

Messages can be left with the reception staff if an Intake worker is not available.
One of Intake Workers will call you back as soon as possible however you will need to advise the receptionist if an urgent response is required, as at times it may take up to 3 days before we can return your call.
