Early Intervention in Chronic Disease

Knox is one of 18 community health services around Victoria to have received funding from the Victorian government to develop our work with people who have chronic or complex conditions.

The objectives of the Early Intervention in Chronic Disease initiative are:
• provision of person-centred care
• recognising consumers are active partners in the management of their chronic disease
• increasing choice and control.
• provision of the right care at the right place and the right time.
• proactively promoting health.
• targeting subgroups of greatest need.
• building a whole of service system response.


Active Living

The Active Living Team is a multi-disciplinary team who predominantly provide services to people in their home.
Services are provided by occupational therapists, physiotherapists, podiatrists and allied health assistants who work together with the individual to maintain and improve their health and wellbeing. By providing proactive initiatives and treatments, the Active Living Team’s ultimate goal is to maximise the individuals quality of life.


Healthy Connections

The Healthy Connections Team provides arrange of services that are predominantly based at Knox Community Health Service or in community venues. Services are provided to the community by physiotherapists, podiatrists and allied health assistants. In addition to clinical services the Healthy Connections Team provide group programs that promote the retention & development of physical mobility and confidence. Group activities also provide social networks for people that may have been otherwise socially isolated.


Specialist Services

The Specialist Team provides clinical services that address specific health conditions. The team is represented by Diabetes Nurse Educators, Cardiovascular Nurse Educators and Dieticians. Community members can get advice on how to manage diabetes and cardiovascular disease on a one to one basis as well as group information sessions.

The dieticians can provide advice to the community on good nutrition, managing obesity, and developing a diet that complements health objectives associated with many chronic and complex diseases.
